Job Summary:

The Community Worker functions as a healthcare advocate performing various patient care activities.  The Community Worker also performs outreach and education in community settings. The Community Worker is responsible for providing education and social support as outlined in the specified program. within established parameters, and consults with supervisor as questions arise for Mile Square Health Center (MSHC) or the Women, Infants, and Children (WIC) Office.

Key Responsibilities: 

  • Host, participate, coordinate, and sponsor community functions to help raise awareness of Mile Square Health Center programs (i.e. school report card pick-up, health education sessions, etc.)
  • Provide educational information and outreach to community residents and assist them with making decisions about health lifestyles and healthcare needs. 
  • Conduct home visits in the community to help identify and recruit potential new patients. 
  • Assist patients in the health market place enrollment process requiring certification as a Certified Application Counselor.
  • Schedule patient appointments via the practice management system or other system as indicated. 
  • Work with staff and community residents to assess their community for strengths, problems, and concerns relative to healthcare (i.e. conducting surveys).  Identify and assist in establishing services needed within the community.
  • Liaison between the institution or agency and the individuals served; refer patients to support services when needed. 
  • Manage initial call intake for MSHC WIC Office; and route calls and messages to appropriate individuals/departments for resolution.
  • Respond to inquiries from clients in order to provide or clarify information on departmental services, policies, and procedures.
  • Collaborate with other state WIC offices to ensure continuity of services; provide referrals and outreach to other appropriate community services and agencies. .
  •  Draft letters and documents on behalf of patients (i.e. verification appointment letters) and the WIC Office.
  • Advocate and promote breastfeeding education; provide basic breastfeeding assistance and information; refer patients to Breastfeeding Counselors if additional support is needed.
  • Participate in orientation of new patients to MSHC; ensure patient receives materials and information pertinents to MSHC and state policies; i.e. hours of operation, programs/services offered, etc.
  • Participate in cross-functional continuous quality improvement and corporate compliance teams; consult with related departments to coordinate seamless access to services.
  • Collect, automate, and evaluate primary performance data, including conducting surveys for ongoing departmental studies; prepare and distribute statistical reports for review.
  • Receive, log, and appropriately route complaints to management in the event a customer service issue cannot be resolved. Assist in conducting primary research on complaints received.
  • Manage file maintenance for the WIC office; conduct inventory and order supplies; attend conferences, workshops, seminars and other WIC functions on behalf of the WIC Office at MSHC.
  • Perform other related duties as assigned.

Minimum Qualifications:

  1. High school diploma or equivalent.
  2. One (1) year (12 months) of experience (paid or unpaid) in work related to the job to be performed.

 

Personal Attributed Needed To Undertake Job 

  1. One year experience within a WIC program providing nutrition to mother and baby

  2. Familiarity with the community as defined by the specific program

  3. Skill in dealing with people from varied ethnic/social/economic backgrounds

  4. Communications skills

Preferred Qualificatons:

• Ability to work extended hours as needed.
• Excellent communication skills.
• Previous or current WIC participation in the community breastfeeding experience of at least 6 months
