Essential Duties:

  • Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
  • Facilitate appointments for consults and support services within established service standards, including social workers, dietitians, physician, etc.
  • Engages and obtains information from patient and/or family to develop and support an appropriate patient educational plan and resources.
  • Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources.
  • Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes and support Tumor Board activities.
  • Deliver quarterly updates to Senior Director of Clinical Operations, and other groups with demonstrate outcomes and performance improvement activities. Adhere to established productivity and quality benchmarks as well as NCCN Guidelines.
  • Responsible for outreach efforts to establish and maintain positive working relationships with key stakeholders (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff, etc.).
  • Invites cancer patients and their caregivers to become part of the cancer survivor community through such venues as the UI Health Cancer Center and its affiliates.
  • Completes required orientation program, organizational and unit-specific competencies in a timely manner.
  • Shares information and collaborates with other members of the health care team to promote effective patient care.
  • Demonstrates a commitment to UI Health values through teamwork and effective communication in an effort to achieve quality outcomes, patient safety, customer satisfaction and a clean, safe and therapeutic environment.
  • Performs other duties as assigned.

Professional/Leadership           

  • Advocates for patients, families, and their significant others, the community, and the nursing profession.
  • Integrates theoretical knowledge, evidence-based research, current standards of practice and clinical experience to provide “quality” patient care as measured by established nursing quality standards and other governing/accrediting health care bodies.
  • Recognizes and supports Shared Leadership as the selected method of nursing governance
  • Participates in organizational, departmental and shared leadership activities; supports department and unit goals in daily behavior.
  • Demonstrates accountability for job performance by: (1) seeking opportunities to enhance job skills and knowledge; (2) maintaining conditions of employment (LPN licensure, BLS and other required unit-specific certifications).  

Organizational

  • Adheres to the UI Health Code of Conduct.
  • Treats patients/families, staff, students and visitors with respect and dignity.
  • Adheres to Nursing’s time, overtime and attendance policies. 
  • Completes electronic learning modules, annual training compliance, and related programs as assigned by UI Health by established date.
  • Complies with University Health Services TB, flu compliance and fit-testing requirements.
  • Utilizes established communication channels/chain of command to escalate or resolve patient care problems.
  • Identifies recurring clinical practice and/or related unit problems and participates in problem solving and resolution.

Education/Research

  • Collaborates with other nurses to identify, promote and provide positive learning experiences for staff and students.
  • Serves as a resource to other nursing staff and students.
  • Accesses UI Health intranet for relevant communication and to review and interpret changes in policy, procedure and other information pertinent to practice.
  • Supports nursing and multidisciplinary research efforts conducted on the unit and in UI Health. 
  • Takes responsibility for continued professional growth/development by identification of learning needs and initiates strategies to meet such needs.

Must be graduate of an accredited LPN program with current License Practical Nursing license from the State of Illinois required.

Required current AHA BLS/CPR and other required unit-specific certifications

At least six months of nursing experience in a health care faculty (such as hospital, nursing home, medical clinic, prison, mental health facility, or closely related facilities).

Oncology experience strongly preferred; bilingual Spanish-speaking skills preferred.

Demonstrated effective communication skills (oral and written) that facilitate therapeutic relationships between patients, families and health care providers

Basic computer skills with the ability to become proficient in electronic medical record (EMR) with on-the-job Training

Excellent attendance, interpersonal skills and work history required.
