Job Summary

The University of  Illinois at Chicago Department of Pediatrics, CHECK program is seeking to fill Medical Social Associate positions to support the Blue Cross Blue Shield Health Equities Pilot Project (BCBS - HEPP). Under administrative supervision from Director, Care Coordination, these positions perform professional social service case management functions in a physical and behavioral health care coordination program. 

 

Essential Duties  / Responsibilities 

  • Assists in the planning, development, and implementation of social and/or educational programs for CHECK Program within the Department of Pediatrics
  • Interviews and assesses the needs of individuals to be served by the programs via phone screening, face to face contact, and other methods utilizing established program assessment tools and procedures.
  • Develop patient individualized plans of care, monitor progress, and coordinate care with patient providers (internal and external)
  • Provides brief intervention services, when needed, following CHECK program protocols and operating within scope of training. For individuals in behavioral health specific positions, this may mean providing brief psychoeducational interventions or responding to patient crisis within CHECK protocols.
  • Provides specialized training and case management for individuals participating in the program; through a combination of telephone, in-person, and “virtual” contact as determined based on program need.
  • Acts as liaison between the institution and the individuals to be served; submits documentation to state agencies, partner organizations, and other funding sources on behalf of the program as needed.
  • Collaborates with other professional staff members in meeting the individual needs of program participants; as necessary, consults with higher-level care providers and physicians.
  • Prepares and maintains necessary records and reports for compliance, reviews, and amendments on an as needed basis
  • Completes, reviews, and submits medical documentation for the purpose of documenting services and determining eligibility for new and continued services.
  • Engages in care coordination related activities associated with program contracts (i.e. identifying and assisting in coordination of addressing patient care gaps)
  • Develops resource materials and protocols for the CHECK team when needed
  • Assists in overseeing and training support staff and student employees as assigned.
  • Performs other related duties and participates in special projects as assigned.

 

Minimum Acceptable Qualifications Required

  •  Bachelor's degree with a major in the social or behavioral science or related health specialty from an accredited college or university.

OR

 Master's degree in social or behavioral science or related health specialty area from an accredited college or university

  • One (1) year (12 months) of progressively more responsible full-time experience in the practice of social services/ agencies or medical/clinical setting maintaining acceptable standards of social work practices

 

Preferred Qualifications

  • Work experience and or training providing knowledge of human behavior as demonstrated through experience in fields of social service, psychology, public health, or customer service settings.
  • Knowledge of basic behavioral and physical health concepts. 
  • Knowledge of best practices for working with program population. 
  • Knowledge of HIPAA and privacy practices. 
  • Knowledge of basic medical / mental health terminology.
