Ambulatory Care Pharmacy Services, a Unit of the Department of Pharmacy Practice at the University of Illinois at Chicago College of Pharmacy, is looking for a Clinical Faculty (Open Rank)/Clinical Pharmacist to join our Specialty Pharmacy Services Group at our Wood Street Pharmacy.  Wood Street Pharmacy is an accredited specialty pharmacy through Utilization Review Accreditation Commission (URAC) and Accreditation Commission for Health Care (ACHC).

Clinical faculty responsibilities of the position include the provision of didactic teaching and clinical experiential teaching/training in the College of Pharmacy’s Doctor of Pharmacy Program and engagement in productive scholarly activity.

Clinical Pharmacist responsibilities include providing clinical pharmacy services to patients seen at the University of Illinois Hospital & Health Sciences System and Wood Street Pharmacy Specialty Pharmacy Services.  The selected individual will implement the specialty pharmacy patient management program to ensure patient access to specialty medications. This service is based in the specialty pharmacy call center in Wood Street Pharmacy with a team of pharmacists, pharmacy technicians and pharmacy students.  Duties include developing and documenting specialty pharmacy care plans, performing treatment initiations, providing patient education and training, performing and documenting clinical interventions such as adverse event management and adherence management, monitoring treatment outcomes, communicating with clinic providers, and documenting in the electronic medical record. Clinic responsibilities include direct patient management and oversight in a designated outpatient specialty clinic.

Minimum Required Qualifications:

  • Candidates must be licensed or eligible for Illinois licensure as a professional pharmacist.
  • Candidates must be a graduate of an ACPE accredited College of Pharmacy PharmD program.
  • Candidates must have completed an ASHP accredited residency training.
  • Candidates must possess knowledge and skills such that they qualify for a non-tenured clinical faculty position with the Department of Pharmacy Practice at the UIC College of Pharmacy.

Preferred Qualifications:

  • Knowledgeable to resolve medication related problems and/or insurance issues.
  • Experience in specialty pharmacy and ambulatory care pharmacy setting.
