UI Health is seeking to hire a Director, Case Management. This position is responsible for the day-to-day operation of nursing unit(s). Oversees comprehensive health care as determined by the scope of practice guidelines and specialty. Manages the nursing practice, including the delivery of patient care, discharge planning, utilization management, technical services, quality, safety, planning, organizing, staffing, and clerical practices.

Strategic Planning and Goal Setting

Determine, plan and establish departmental strategic course and the provision of utilization management and discharge planning to ensure safe, timely, efficient transitions of care, throughput, and improved patient outcomes across the health system. Analyze changing demographics, increasing competition and other social, economic and political factors in the organization environment to review and adjust the department vision and values statements. Outline and modify strategic goals and ways to accomplish them. Identify environmental impediments to client or patient progress.

Manage from pre-admission through discharge including monitoring patient care, addressing patient and family needs, resolving obstacles and problems, planning for patient discharge.

Establish, implement and evaluate quality of care. Plan and develop patient safety innovations and practice improvement.

Finance / Budget Management

Formulate and justify the department’s annual budget and capital projects and ensure that budget is administered within the hospital guidelines.

Review invoices to verify the service is appropriately funded by the department and paid on time.

Prepare budget for assigned area or program and is responsible for adherence to approved budget.

Regulatory Compliance

Develop and maintain hospital and departmental policies and work practices and procedures to ensure adhere to current regulations and best practices in accordance with Centers for Medicaid and Medicare Services (CMS), The Joint Commission, and Illinois Department of Public Health requirements.

Monitor and evaluate operations for compliance with licensure, accreditation and regulatory standards.

Prepare program audits for the State and other regulatory bodies as required.

Patient Consulting

Develop a therapeutic relationship with patients, family members, visitors and colleagues.

Interpret and direct the implementation of nursing philosophy and objectives, departmental policies and procedures, and apply recognized standards of nursing care and practice.

Staff Management

Interview, hire, counsel, orient, assign, evaluate and mentor staff to meet departmental and patient needs, and health system objectives.

Determine personnel, material, equipment, and facilities needed to accomplish program goals. Assigns, directs and reviews the work of subordinate supervisors/other nursing personnel

Integrate shared leadership and foster collaboration across multi-disciplinary disciplines to arrive at decisionsaffecting the unit.

Assign, direct and review the work of subordinate supervisors/other nursing personnel. Prepare formal answers to, and recommend the adjustment of, grievances and disciplinary matters.

Lead subordinates to enable them to make decisions and cope with challenges associated with social counseling.

Plan, select and devise nursing operations, work methods, procedures, work flow and standards for quality andquantity of work, including staffing standards.

Trains and mentors supervisors and managers.

Performs other related duties and participates in special projects as assigned.

 

  • Illinois Registered Nursing license required.
  • Bachelor’s and/or Masters of Science Degree in Nursing required.
  • Master’s degree preferred.
  • Minimum of five (5) years Utilization Management experience required.
  • Three (3) years management experience is preferred.
  • Preferred candidate has experience working with both providers and payor sources
  • Exceedingly strong verbal and written communication skills to maintain effective working relationships with departments, employees, managers, patients/families, regulatory agencies, other Medical Centers, vendors, auditors.
  • Leadership, management, and organizational skills are essential.
  • Ability to handle employees’ stress associated with the intense clinical and/or patient experience.
  • Ability to plan and administer a large-scale service program; analyze situations accurately and adopt effective courses of action.
  • Practices confidentiality and maintains sensitive information.
