Job Summary:

UI Health is seeking two (2) Medical Social Consultants (Inpatient Medical Social Work) to join our Health Social Work team.

This position will report to the Director of Health Social Work and take direction from leadership under the Department of Health Social Work. Assists in the development, direction, organization and coordination of various programs within the scope of Social Work, i.e. Quality Assurance; coordinate activities for the development of the department; participates in planning departmental goals and implementing policies and procedures; assists in the development of new programs; also provides direct patient care service, staff supervision, consultation and serves on multidisciplinary planning committees and engages in related duties as assigned.

Key Responsibilities & Duties:

Primary Duites:

  • Ability to exercise independent judgment, understand and follow complex oral and written instructions
  • Participate in the development of departmental goals, objectives, policies and procedures, and participate in their implementation and monitoring for the purpose of comprehensive service delivery.
  • Participants in the planning and implementing of in-service training programs in all area of staff development.
  • Multidisciplinary team collaboration and planning – collaboration and/or discussion, which shares specialized knowledge with other health care personnel regarding patient psychosocial problems, procedures, services and participation in development of a comprehensive care plan.
  • Supervision of students – Organize the onboarding, provide oversight of supervision and training of social work student interns.
  • Submits reports, statistics and other materials to the Director as required.
  • Participates in staff meetings and conferences within the department.
  • Must possess the ability to work with people in a leadership role. This will require effective communication and administrative skills. The ability to obtain the cooperation of others, as well as to develop and direct cooperative team activity. Ability to recognize and respond to emerging needs of the department, staff and patients. Must be willing to assume other duties as required for the day-to-day operation of the department.
  • Ability to exercise independent judgment, understand and follow complex oral and written instructions, implement a sophisticated filing system and communicate effectively with a wide variety of individuals.
  • Methods Used: Group Interview, observations, staffing surveys, peer review, peer benchmarking
  • Provide Referrals for Community Resources
  • Takes on a Leadership role on Departmental Committees
  • Represents the Director in Meetings by providing feedback, consultation and/or reports for decisions and actions to be taken.
  • The following services or interventions shall be provided and not limited:
    • Assesses patients and families psychosocial risk factors to include prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
    • Assess for Depression, suicide screening, Depression Scale
    • Assess, Advocate and provide for Domestic Violence and Addiction Needs
    • Provides intervention in child abuse/neglect, domestic violence, elder abuse, institutional abuse and sexual assault.
    • Facilitates application for entitlement programs including Social Security, Public Aid, Worker's Compensation and Disability.
    • Provides for coordination of services with community agencies. Maintains a current resource file for all patient care areas.
    • Addresses patient issues related to advanced directives. Coordinates referrals and support for patients needing hospice care.
    • Facilitates communication of medical information in the context of the family system.

Additional Duties Include:

  • Provide same but not limited services as identified above to none non assigned areas as called on to cross cover when needed.
  • Work with a diverse population in a Medical Setting.
  • On-Call duties to provide phone consult to all hospital areas after normal business hours. During disaster the on-call person would help activate phone triage list to arrange help as needed during an internal or external disaster.
  • Provide Cross Coverage to other clinics and inpatient medical units.
  • Provide supportive services and resources for possible termination of pregnancies.
  • Direct and Indirect Supervision Received-This person works independently on a day to day basis in regard to direct supervision received. The Director-Department of Health Social Work provides direction and guidance in matters relating to social work care. The objectives and boundaries for the duties and responsibilities of this position are defined collaboratively by the employee and Director-Department of Health Social Work. The employee and Director-Department of Health Social Work will meet on a regular basis so that support, guidance and direction can be given. This employee is evaluated periodically in verbal exchange and at least annually in writing.
  • Other responsibilities (not included above) which may be assigned by the Director to meet the specific needs to the department, such as, preparation for TJC, IDPH, CMS and other accreditation programs for the purpose of quality assurance and compliance.
  • Other duties as assigned.

Minimum Acceptable Qualifications Required:

  1. Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.
  2. Current State of IL Licensure as a Licensed Social Worker (LSW) or a Licensed Clinical Social Worker (LCSW).

 

Preferred Qualifications:

  • Current State of IL Licensure as as Licensed Clinical Social Worker (LCSW)
  • A certificate as an Accredited Casemanager or ACM Preferred at time of application
  • Bilingual language skills with the ability to read, write, speak and translate Spanish/English
  • Minimum of 6 years of experience in a Medical field of Social Work as a licensed Social Worker.
  • Experience with provding both inpatient and outpatient services for both children and maternal populations within a NICU, Pedatrics, and/or Center for Women's Health settings.
