The University of Illinois & Hospital & Health Sciences System (UI Health) Mile Square Humboldt Park is seeking a Community Affairs Specialist I to join our team. The Community Affairs Specialist I functions as a healthcare advocate performing various patient care activities. They are responsible for providing outreach, education, and social support in community settings as outlined in specific programs.

The work schedule for this position will be flexible shifts to accomodate community events (i.e. evenings, early mornings and weekends but mostly weekdays).
 

 

 

Job Requirements and Duties:

  • Assist in the development of programs that will coordinate MSHP services and community activities toward improving human and environmental needs of numerous groups and organizations throughout the community.
  • Assist in the planning and implementation of policies, programs, and procedures of Mile Square Health Center - Humboldt Park services, as to assure their adaptability and responsiveness to common problems and concerns of the community.
  • Serve as a resource for participants in our programs; and develop relationships with healthcare providers and other community-based service providers.
  • Conduct community-based visits to help identify and recruit potential new patients and potential community partners.
  • Perform home visits for the purpose of providing education, support, home health assessments, obtaining competing consents, follow-up on provider referrals, and assisting families in addressing barrier to health care access.
  • Work toward establishing educational, cultural, social, and recreational activities between the Mile Square Health Center - Humboldt Park, community partners and the public.
  • Schedule patients appointments via the practice management system or other system as indicated.
  • Liaison between the institution or agency and the individuals served; refer patients to supportive services when needed and follow up on referrals; and assist participants in utilizing referral services.
  • Assist in ambulatory receptionist duties.
  • Educate and informs patients of UI Health patient portal, registers and/or assist patient with patient portal registration.
  • Assist in the development of resource materials and the preparation and maintenance of records.
  • Perform other related duties an participate in special projects and assignments as assigned.

Minimum Acceptable Qualifications:

  1. High school graduation or equivalent.
  2. Three years of experience in public contact with an agency or organization that is oriented toward community activities.

Preferred Qualifications:

  • Fluent in Spanish (preferred)
  • Familiarity with the community as defined by the specific program.
  • Experience with working with marginalized populations and varying economic and social backgrounds.
  • Certified Application Counsel
