The University of Illinois Hospital & Health Sciences System (UI Health) is seeking a a full-time Program Service Specialist (Chronic Disease Navigator and Community Health Coordinator) functions as a healthcare advocate performing various patient care activities including social service coordination in clinics, schools, communities, and homes. He/she is responsible for providing outreach, education, and social support in community settings as outlined in specific programs.

The work schedule for this position is flex days with some Saturdays included.

Job Requirements and Duties:

  • Host, participate, coordinate, and sponsor community functions to help raise awareness of cancer screening services to meet the project goals.
  • Provide educational information and outreach regarding programs, associated requirements, and other opportunities to community residents and the general public. Assist them with making decisions about healthy lifestyles and healthcare needs.
  • Serve as a resource person for participants in the program; develop referral relationships with healthcare providers and other community based service providers.
  • Conduct community-based visits to help identify and recruit potential new patients and potential community partners
  • Conduct outreach/in-reach activities, hold outreach events and will disseminate fliers, fact sheets, brochures and posters targeting specific groups. Procure ads and distribute incentives with educational messages.
  • Perform home visits as needed for the purpose of providing education, support, home health assessments, obtaining and completing consents, follow-up on provider referrals, and assisting families in addressing barriers to health care access.
  • Work with staff and community residents to assess their community for strengths, problems, and concerns relative to healthcare (e.g. conducting surveys, interviewing, etc.). Assess, identify, and assist in establishing services needed within the community.
  • Navigate clients through the healthcare system to determine the insurance status, need of each client, and navigate them through the health system accordingly.
  • Assist patients with the health market place enrollment process
  • Schedule patient appointments via the practice management system or other system as indicated.
  • Liaison between the institution or agency and the individuals served; refer patients to support services when needed and follow up on referrals; and assist participants in utilizing referral services.
  • Create and develop resource materials and the preparation and maintenance of records and data.
  • Complete all required reports to document activities and provide data to measure impact.
  • Perform other related duties and participate in special projects and assignments as assigned.

 

Minimum Acceptable Qualifications:

Any one or any combination of the following types of preparation:

  1. Credit for college course work comparable to that leading to a major in social work or one of the social or behavioral sciences.
  2. Progressively more responsible social service/counseling work experience at a paraprofessional level under qualified supervision that totals 1.0 unit according to the following conversion rates:
  • 120 semester hours (or Bachelor's degree) of "A" = 1.0 unit
  • Three years of "B" = 1.0 unit 

Amounts of education and experience less than those listed above should be converted to decimal equivalents of 1.0 unit and added together when computing combinations of the different types of preparation.?
