~~The Transplant Coordinator Clinical Nurse Consultant II is based out of Rockford IL and is accountable for facilitating the delivery of patient care utilizing the Nursing Process and Relationship-Based Care model as the framework. The Clinical Nurse Consultant II provides patient care and consultation, coordinates the educational development of staff members and acts as a role model in clinical practice. The CNCII strives to improve patient outcomes by integrating Shared Leadership, Evidence Based Practice, Relationship Based Care and standards of nursing excellence into clinical practice. Through therapeutic relationships with patients, families, self and colleagues, the Clinical Nurse Consultant II participates in monitoring, developing, and evaluating nursing practice while fulfilling unit and organizational goals.

The Transplant Coordinator (CNCII) reflects the mission, vision, and values of the University of Illinois Hospital, adheres to the organization 's Code of Ethics, Compliance Programs, all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

The Transplant Nurse Coordinator (CNCII) is a Registered Nurse with experience in and knowledge of transplantation and living solid organ donation.  Practicing under the supervision of transplant surgeons and physicians, the transplant Nurse Coordinator ensures coordination of the clinical aspects of transplant patient and living donor care throughout the pre-transplant phase.  The job duties vary with the particular setting or assignment.  Transplant Nurse Coordinators can be assigned to kidney, pancreas, liver and/or small bowel programs.

RN license required.

Two years of current practice as a registered nurse required

Minimum of Bachelors in Nursing required. MSN strongly preferred.

Previous transplant or organ specific experience preferred

Fluent in Spanish preferred.

Transplant or Organ specific certification preferred  (may obtain within 2 years of employment)

Excellent attendance, interpersonal skills and work history required.
