The Community Health Worker – Contract Tracer responsible for communicating with persons who have had sustained close contact with a COVID-19 infected individual and providing them with information and guidance to interrupt/reduce ongoing transmission of the disease. Tracers will help obtain symptom information; refer contacts for testing and/or supportive services; provide instructions for self-quarantine; and obtain information about the contacted individual’s contact with others.

The objective of our grant is to address the social disparities within Dearborn Homes.

**Candidates residing in the Dearborn Homes area are highly preferred as this position will primarily be working in this area.**

Assist in the development of resource materials and the preparation and maintenance of records and data.

Perform intake interviews for potential COVID-19 positive cases

Document interviews

Work with the team to execute the next steps and notifying close contacts

Provide educational information and outreach regarding programs, associated requirements, and other opportunities to community residents and the general public. Assist them with making decisions about healthy lifestyles and healthcare needs.

Serve as a resource person for participants in the program; develop referral relationships with healthcare providers and other community based service providers.

Conduct community-based visits to help identify and recruit potential new patients and potential community partners.

Work with staff and community residents to assess their community for strengths, problems, and concerns relative to healthcare (e.g. conducting surveys, interviewing, etc.). Assess, identify, and assist in establishing services needed within the community.

Liaison between the institution or agency and the individuals served; refer patients to support services when needed and follow up on referrals; and assist participants in utilizing referral services.

Perform other related duties and participate in special projects and assignments as assigned

MINIMUM ACCEPTABLE QUALIFICATIONS

  1. One year of experience (paid or unpaid) in work related to the job to be performed
  2. Contact tracing experience preferred
  3. High School Diploma Preferred

PERSONAL ATTRIBUTES NEEDED TO UNDERTAKE JOB

  1. Familiarity with the community as defined by the specific program
  2. Skill in dealing with people from varied ethnic/social/economic backgrounds
  3. Communications skills

Amounts of education, experience, or training less than those listed above should be converted to decimal equivalents of 1.0 unit and added together when computing combinations of the different types of preparation.
